Partnering with your heart healthcare team
Depending on your situation and where you are being treated, your heart failure care team may consist of some or all of the following medical professionals. So, as you can see, you don’t ever have to feel like you are all alone in this.

Play a central role in your heart failure care, coordinating services with other members of the care team.
Specialise in diagnosing and treating heart problems.
Provide care by performing exams, taking medical histories, ordering tests and treatments, and making referrals to other specialists. These professionals will often be your point of contact for reporting warning signs of heart failure.
Responsible for the long-term monitoring of patients with heart valve disease, replacement valves, or implantable cardiac devices. They also carry out cardiac tests, such as echocardiograms, Holter monitors, and blood pressure measurement.
Help heart patients strengthen themselves through exercise and nutrition.
Trained to help you understand diabetes and what you need to do to manage the condition and maintain or lower blood sugar levels.
Trained to evaluate your diet and suggest heart-healthy changes in your eating that may help reduce your cholesterol level, lower blood pressure, and manage diabetes, if you have it.
Psychologists and counsellors can provide individual and group counselling and help you identify support groups. They can also help you adjust to your new condition and identify helpful coping strategies.
Fill prescriptions for medication and can answer questions about drug interactions, insurance, generic medications, and much more.
The cardiologist was first. Then, the heart failure team of specialists (nurse, cardiologist) work with patients to improve and make lifestyle changes, direct to psychologists for emotional health, and all these important notes—along with the treatments and how they interact with the treatment is very helpful."
